Dr. Shawna doesn't take insurance because that limits what treatment she can do for her clients.
If you'd still like to use insurance (submitting it yourself), please note that state law requires a doctors note after 10 visits. If you don't get one after 10 visits, Dr. Shawna will no longer be able to see you. Please let us know prior to the first visit if you want to submit to insurance yourself as this will require us to code things differently.
You can have your Primary Care Doctor email or fax the note over.
Our software requires us to charge a 3% card fee every time a credit/debit card is used to pay for appointments. (For example: $300 is $309)
If you don't want to pay this fee, we recommend you use either; Check, Cash, or Venmo (there will be a QR Code at the front desk)
